How are out-of-network claims processed?

For employers and groups who choose a standard or simple plan any out-of-network claims reimbursements are similar to those of traditional insurance companies. Dental providers submit a standard ADA claims form, completed, to Bento via fax or mail and claims are processed upon arrival.
